OSHA cites Alabama contractor after sewer gas exposure kills two workers

ConDig (20-Feb-25) The Occupational Safety and Health Administration (OSHA) has cited an Alabama contractor following an investigation into a deadly confined space incident at a Mobile worksite that left two workers dead.

The agency found that employees of Construction Labor Services Inc. were exposed to hazardous sewage gases on Aug. 11, 2025, while installing and repairing sewer lines inside a manhole.

According to federal investigators, three workers entered the manhole and were overcome by sewer gas, rendering them unresponsive. One worker regained consciousness and was able to self-rescue. The other two workers died from their injuries.

OSHA determined the employer failed to implement required confined space safety measures. The agency said the company lacked a confined space entry program, proper procedures and training, and adequate emergency response plans. Investigators also found the employer did not have processes in place to determine acceptable entry conditions before workers entered the confined space.

As a result of the investigation, OSHA issued 16 serious violations to Construction Labor Services Inc. and proposed penalties totaling $257,707.

The company has 15 business days from receipt of the citations to comply, request an informal conference with OSHA’s area director, or contest the findings before the independent Occupational Safety and Health Review Commission.
